Wellbeing Worker - Health Team North (6 Month FTC)

Carers Support West Sussex provides services for and with unpaid carers who look after a relative or friend who could not manage without their support. Our teams provide tailored information, advice and support to help improve carers’ resilience and ensure they are supported to maintain their caring roles.
We are currently offering a fixed term role within our Health Team, working with unpaid Carers as part of a wider team. You will be working within a hospital setting one or two days a week and also working from home. You will be readily able to travel across the county of West Sussex for occasional training and meetings.
You will have experience of working with people, perhaps within a health or social care background and knowledge of the challenges facing unpaid carers from all backgrounds, with the ability to identify and address their individual and collective needs.
You will have excellent interpersonal skills, able to advocate for people, communicate clearly and with cultural sensitivity to a diverse audience of service users.
This is a 6-month fixed term contract with a possible extension, within the Health Team, north and east of the county. The role is for 37 hours per week, but 30 hours would be considered for the right candidate.

Purpose
- Work as part of the Health Team which offers support with and for carers where they or the person they care for are in a hospital setting.
- Work as part of the team to have a regular presence at Princess Royal, (Haywards Health), East Surrey Hospital (Redhill), Royal Sussex County and associated Community Hospitals.
- Through partnership working with hospital staff, be pro-active in the identification and subsequent support of carers.

About You
- A good knowledge of health and social care, particularly as it relates to carers and adults
- A natural communicator.
- A strong commitment to equity, diversity, and inclusion practices.
- Excellent organisational, planning and time management skills and ability to make decisions
- Confident, energetic and passionate about improving the wellbeing of others
- Work in a way consistent with the organisation’s culture and values
Experience
- Experience of working within a health setting would be beneficial but not essential
- Excellent interpersonal skills, able to work collaboratively, connect and build positive relationships
- Experience in delivering training to a range of professionals.
- Highly competent in use of IT
- Ability to work within a hospital setting and readily travel, including rural locations and out of office hours.
Key Duties
- Provide tailored, personalised information, guidance and support to support carers through their hospital journey with a focus on supporting discharge.
- Research and explore options to empower carers to make informed decisions and find solutions which work for them to achieve their goals.
- Work collaboratively with health, social care and voluntary sector partners, seeking out opportunities to work together to identify and enhance support available to carers within a hospital setting.
- Accurately record all interventions on the database in a timely manner.
