Working together, we aim to make it as easy as possible for you to identify carers in primary care, support their health needs and refer them to us for wider practical and emotional support.

This portal provides our primary care partners with a dedicated GP referral route, quick links, and key resources to embed carer friendly practice, upskill staff and connect with us.

Primary care professionals have a crucial role in identifying carers early in their caring journey and connecting them to targeted support. Please seek consent and refer carers to us here.

Carers Lead Network

The Network is a closed group for named carers leads in West Sussex who are responsible for carers within their GP surgery. Meetings include focus topics, practice spotlights, disseminating information and resources, networking, and learning from one another. Contact the Team for more information.
