Long Term Conditions

Caring for someone with a long-term condition can be very demanding emotionally and physically, particularly as many people can be coping with multiple diagnoses. Because everyone’s situation is different, we offer personalised information and guidance to help you in your caring role, and to help you to maintain your own health and wellbeing whilst caring for others.

What is a Long-term Condition?

A long-term condition is a physical health condition that can require ongoing management with medication or other treatments over a period of years. This applies to a wide variety of conditions, but the commonly known long-term conditions include the following:

  • Acquired Brain Injury
  • Cardiac (heart) conditions
  • Cancer
  • Chronic dermatological conditions such as Psoriasis and Eczema
  • Diabetes
  • Chronic gastrointestinal conditions such as Crohn’s and Colitis
  • Chronic haematological (blood) conditions such as Pernicious Anaemia, Sickle Cell, and Haemophilia
  • Kidney disease
  • Liver disease
  • Chronic musculoskeletal conditions such as Fibromyalgia, Osteoporosis, and Arthritis
  • Neurological conditions such as Epilepsy, Chronic Migraine, Parkinson’s, Multiple Sclerosis, Motor Neurone Disease, and Strokes
  • Physical disabilities
  • Chronic respiratory conditions such as Asthma or Chronic Obstructive Pulmonary Disease (COPD)
  • Sensory loss such as sight or hearing impairment

Open-to-All Carer Groups

Meet and share experiences and support with other carers. Our open-to-all carer groups are an opportunity to talk about your caring situation and gain further support from other carers, as well as support from wellbeing workers from Carers Support West Sussex
